What are the responsibilities and job description for the Diabetes Educator / Dietitian position at MedStar Medical Group?
Educates, supports, and advocates for people affected by diabetes, addressing the stages of diabetes throughout the lifespan. Promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.
Education
Bachelor's degree in Nursing, Dietetics, or nutrition related field required
Experience
Prior work experience not required Diabetes self-management education and support experience preferred
Licenses and Certifications
REG DIET - Dietitian - Registered Valid license in Maryland as a Dietitian (must be registered by the Commission on Dietetic Registration) required or
RN - Registered Nurse - State Licensure and / or Compact State Licensure Valid license in Maryland as a Registered Nurse required or
Licensed Nurse Practitioner Valid license in Maryland as a Nurse Practitioner (by the American Nursing Association) required and
CDE - Certified Diabetes Educator Certification If not already CDE will obtain certification within 1-1 / 2 Yrs required
Knowledge, Skills, and Abilities
Verbal and written communication skills.
Basic computer skills including the use of Excel are preferred.
Highly organized and strong attention to detail.
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
Collaborates with nurse practitioners, primary care providers, and endocrinologists in developing patient-centered diabetes plan. Confers with Physician(s) and / or Nurse Practitioner(s) concerning assigned patients.
Communicates / meets with the program leader regarding the progress of program implementation at MMG practices and concerns regarding patient care.
Completes patient's diabetes assessment and develops educational goals and a diabetes self-management support care plan based on patients' assessed needs.
Provides quality diabetes self-management education and medical nutrition therapy to patients and families in individual and group settings. Utilizes appropriate teaching techniques that are sensitive to the learning styles of the patient.
Serves as an educational resource to MMG practice. Provides technical and educational assistance as needed.
Follows patients closely (in person and via phone / video conferencing) until the patient meets program goals or is transferred to the virtual control center.
Maintains required records, reports, and statistics.
Completes timely documentation in the patient's medical record and timely coding of services to optimize patient safety and assure compliant billing practices are followed.
May Provide distance visits by video conferencing, telephone, text, and email.
Partners with MedStar Diabetes Institute and records outcomes in Diabetes Institute database as required.
Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees, and represents the department and hospital / facility in community outreach efforts as appropriate.
