What are the responsibilities and job description for the Community Health Advocate position at medstar-pilot?
Under the direction of the Manager of Community Health Outreach, the Community Health Advocate (CHA) works closely with case management, community and social service agencies to provide connection to resources and programs for community members to improve their health and general well-being. Primarily working in a community-based setting, the CHA serves as a link between health/social services and the community to facilitate access and improve the quality and cultural competences of service delivery. In addition, the CHA builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities including outreach, community education, informal counseling, social and navigation support, health screenings, assessments and advocacy.
