Job Summary
MedTruly is on a mission to help primary care providers deliver team-based behavioral health care.
Our Behavioral Health Care Manager provides remote, evidence-based interventions via the collaborative care model to promote mental health access in rural and underserved communities. These interventions include behavioral activation, problem solving treatment, motivational interviewing and cognitive behavioral therapy in the small primary care settings. Our BH Care Manager is a core member of a collaborative care team that includes the patient’s PCP and a psychiatric consultant.
Duties
- Provide individualized and evidence-based care to patients
- Offer counseling with clients to address their mental health concerns
- Provide patient education about common mental health and substance abuse disorders and the available treatment options
- Support and closely coordinate behavioral care with the patient’s PCP
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant
- Screen and assess patients via PHQ9, GAD7 and other tools as needed
- Monitor patients for changes in symptoms and treatment side effects
- Support psychotropic medication management focusing on adherence, side effects and effectiveness
- Participate in regular caseload meetings with psychiatric consultant
- Identify social determinant barriers and work with care coordinators to assist
Qualifications
- Licensed Mental Health Counselor; Licensed Marriage and Family Therapist; Licensed Social Worker; or Registered Nurse (BSN recommended)
- Experience screening for common mental health and/or substance abuse disorders
- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate
- Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
- Proven experience in behavioral health management or clinical counseling
- Strong knowledge of crisis management techniques and patient assessment processes
- Excellent communication skills, both verbal and written, with the ability to engage effectively with clients and staff
- Ability to work collaboratively within a team-oriented environment while maintaining a focus on client-centered care
- Familiarity with relevant regulations and standards governing behavioral health practices
