Social Worker

Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong commitment to providing support and advocacy for individuals facing various challenges, including mental health issues, addiction, and developmental disabilities. This role requires effective communication skills, empathy, and the ability to work collaboratively with clients and other professionals in a clinical or community setting. We partner with a few nursing homes around our clinic where we provide clinical services. Our provider sees the patients once a month and we would want to provide mental health services to these patients. You will be required to go to the nursing homes once a month to meet these patients and assess their well being.

Responsibilities

• Conduct assessments to determine clients' needs and develop individualized care plans.
• Provide patient care and support for individuals with autism and other developmental disorders.
• Facilitate group therapy sessions and provide crisis intervention as needed.
• Offer addiction counseling services to clients struggling with substance use disorders.
• Engage in clinical research to enhance treatment methodologies and improve client outcomes.
• Maintain accurate documentation using ICD-10 coding standards for diagnoses and treatment plans.
• Coordinate home community care services to ensure clients receive comprehensive support.
• Collaborate with multidisciplinary teams to ensure holistic care for clients.
• Advocate for clients' rights and access to necessary resources within the community.

Skills

• Strong background in patient care, with an emphasis on working with individuals on the autism spectrum.
• Proficiency in crisis intervention techniques and group therapy facilitation.
• Knowledge of clinical research methodologies related to social work practices.
• Familiarity with addiction counseling principles and practices.
• Competence in coding using ICD-10 standards for effective documentation.
• Excellent interpersonal skills, enabling effective communication with clients, families, and colleagues.
• Ability to work independently as well as part of a team in a fast-paced environment.
• Empathy, patience, and a genuine desire to help others navigate their challenges.
