Job Overview
The primary job of the Case Manager is to lessen the impact to public assistance once a person becomes employed, helping them bridge the gap at maintaining self-sufficiency and providing service and resource through peer mentoring and partnerships with a Care Team.
Case Manager will provide support, education, outreach, and engagement services from a lived experience approach. The Case Manager will provide job readiness skills and needed resources to identified individuals helping them obtain and sustain independence. The Case Manager will lead and coordinated service with the agency’s care team to provide a wraparound service to these identified individuals. This position will assist with bridging the gap between benefits and self-sufficiency and will be able to provide the clients with a support network/mentor by providing support, encouragement, and life skill guidance.
Duties
- The Case Manager will complete all necessary Compassion County/Trauma training.
- The Case Manager will work with and lead MCDJFS Care Team, representing Children Services, Child Support, Income Maintenance, Workforce Development in coordinated efforts to bridge the gap between benefits and self-sufficiency.
- Inputting information received into the Benefit Bridge collection tool for each participant, including invoices, case management services, tracking, etc.
- The Case Manager will work directly with Workforce Development Supervisor in ensuring services are being provided within the parameters of the Benefit Bridge Program.
- Case Manager will provide peer-to-peer coaching and support to clients and/or families. Services may include, but not limited to:
o Help each family develop their own strategy for success and create a plan to support them
o Providing peer support services by engaging families, assisting them in developing and implementing their individual treatment plan, teaching daily living skills such as transportation training and sharing their own journey to inspire and support the consumer in their journey to self-sufficiency.
o Completing assessments to determine identifying barriers to the case plan
o Working with the Care Team in identifying goals, strategies, and resources and collaborate services collectively.
o Understanding community resources and connecting clients to available services to help with overcoming barriers
o Working closely with social service agencies as needed
o Providing effective verbal and nonverbal communication to both clients and care team
Duties
- Develop, implement, and manage comprehensive employee benefits programs, including health insurance, retirement plans, and wellness initiatives.
- Conduct regular assessments of benefit offerings to ensure competitiveness and compliance with legal regulations.
- Serve as the primary point of contact for employees regarding benefits inquiries and issues.
- Analyze data related to employee benefits utilization and trends to inform decision-making.
- Collaborate with external vendors and service providers to negotiate contracts and manage relationships.
- Provide training and support to HR staff on benefits-related matters.
- Ensure compliance with federal and state regulations regarding employee benefits.
- Prepare reports on benefits program performance for management review.

QUALIFICATIONS:
· High School Diploma or equivalent,
· Lived experience of public assistance
Or an equivalent combination of education
Job Type: Full-time
Pay: From $18.18 per hour
