We are looking for an LPN to join our Urbana Family Medicine team as a Care Coordinator!
What You'll Do:
This position supports multiple providers. In this role, you will have the opportunity to help support patients by collaborating with patients to:
• Set healthcare goals
• Complete advance care plans
• Track providers quality goals
What You'll Bring:
- Completion of post secondary program in Medical Assistant OR
- Nursing degree
- Current Ohio LPN license
- Knowledge of Microsoft Office
- Preferred:
- Experience as a physician officesÂ
- BLS/CPR certified preferred.
- Proficient with technology required for data tracking and reporting
