Key Responsibilities
- Conduct thorough assessments of patient needs at admission and throughout their stay.
- Develop and implement effective discharge/transition plans in collaboration with healthcare teams.
- Clinically assess, plan, implement, coordinate, monitor, and evaluate options and services to meet an individual's health needs.
- Collaborate with social workers to identify the need for team and/or patient and family conferences.
- Consult regularly with physicians and multidisciplinary teams to evaluate patient status and appropriateness of medical care.
