Summary
The Patient Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Patient Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care.
Responsibilities
Serves as a liaison between patient and clinical staff, administration, physicians, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Intervenes as appropriate to advocate for patient and family.May collect and assess data to identify quality trends or actual sources of risks to patients and employees. Collaborates with multidisciplinary team and committees with respect to quality outcomes.Continuously reviews and evaluates patient progress as outlined within the plan of care. Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.

Master's in Social Work (MSW) degree preferred if degreed.(RT) and certified in Radiation Therapy (T) by the American Registry of Radiologic Technologists (ARRT) - Radiation Therapy License required if working in Radiation Oncology.Additional Education Info: Graduate of an accredited registered/Professional Nursing Program or MSW degree.Additional Credential Info: Radiation Therapy license required if working in the Radiation Oncology department.
