Job Summary : The Wound Care Licensed Vocational Nurses (LVNs) and / or Licensed Practical Nurses (LPNs) provide clinical care on patients with wounds under the supervision of a registered nurse (RN) or physician. They assess wounds, clean and dress wounds, and apply medications to promote healing. They also educate patients on wound care and how to prevent complications. Actively and consistently contributes to department operations and communications, behaves in a manner consistent with the mission, vision, and values of Pipeline Health, upholding standards of AIDET (Acknowledge, Introduce, Duration, Explanation, Thank you) patient communication.
Essential Functions :
- Identify, manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions,foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic, and viral skin infections, scaling popular diseases, and benign tumors.
- Consult with the Care Planning Team concerning assessments evaluations and assist in planning and developing skin care treatment to be performed for the patient.
- Provide, with established protocols and under the supervision of the registered nurse and / or physician, treatment for uncomplicated dermatologic disorders as directed by the physician.
- Confer with registered nurses and / or other licensed nursing personnel regarding dermatologic disorders of patients.
- Ensure patients with decubitus ulcers receive appropriate prophylaxis and treatment, such as daily inspections, turning and activity, a well-planned diet, and maintaining a clean, dry bed.
- Provide information to patients and family members concerning prevention of common skin disorders and ways to maintain or improve the general health of the patient.
- Document treatment and findings in patient health record.
- Uses clear, concise, professional communication with coworkers, patients, all customers internal and external.
- Uses AIDET in interactions with patients and family members. Utilizes white boards for patient communication.
- Collaborates across disciplines to assist in coordination of patient care, including but not limited to patient transfer, discharge, referrals, and spiritual / psychosocial support needs.
- Acts with a sense of urgency when performing tasks.
- Basic unit / department maintenance such as keeping files, drawers, cabinets free from unnecessary clutter.
- Reports on any equipment and or environmental issues for repair.
- Abides by HIPAA (Health Insurance Portability and Accountability Act) regulations.
- Utilizes SBAR (Situation, Background, Assessment, Recommendation) when communicating with oncoming shift, physician communication and giving report to ancillary or other nursing services.
- Speaks up to stop the line and escalates potential safety events if necessary.
- Completes and attends monthly training assigned.
- May float to other areas of the hospital as needed.
