A Day in the Life of a Resource Coordinator:
The Resource Coordinator provides assessments, resource referral, and collaborative planning for patients within their scope of practice. Their services focus on coping strategies, financial assistance, community resources and assessment and planning for discharge needs.

Here Is What You Will Be Doing:
• Performs psychosocial assessment and screening of patients for discharge needs to include making referrals and monitoring for compliance with/adherence to plan of care.
• Manages individuals with focus on high risk populations across the continuum of care.
• Refers the patient to the appropriate community resources with the patient’s understanding and cooperation. Acts as liaison with community and medical agencies to coordinate appropriate referrals for services.
• Coordinates and participates in family and case conferences. Promotes patient/family acceptance and follow through with treatment recommendations. Facilitates effective treatment of patient’s medical conditions and rate of recovery.
• Provides discharge planning related in-service programs to staff as appropriate.
• Documents all patient communications and coordination in the medical record as appropriate.
• May provide post-discharge visits.
Desired Skills:
• Excellent oral/written communication skills necessary to positively interact with patients, patient’s families, visitors, physicians and other staff
• Ability to function independently and as a team member
• Solid computer skills
• Ability to assess, evaluate, explain, teach, problem solve using critical thinking; and perform calculations
• Ability to drive to and from a variety of settings in varying weather conditions
Here Is What You Will Need:
• Bachelor’s Degree or higher
• Valid USA driver’s license
• Must have clean driving record that follows Cheyenne Regional Medical Center’s driving guidelines
• Cheyenne Regional AHA RQI within 14 calendar days of start date
Nice To Have:
• Bachelor’s Degree or higher in social work, psychology, sociology, or healthcare related field
• Prior Discharge Planning experience in a healthcare related field
