Position Summary:

  • Effectively utilizes hospital resources in the development of care, discharge planning and Utilization Review.
  • Utilization Management performs prospective, admissions, observation and concurrent review according to standard policies and procedure.
  • Utilization Management reviews and analyze processes designed to ensure appropriate utilization of medication, blood products, operative and invasive procedures, and use of resource intensive diagnostic testing and treatment.
  • Discharge Planning assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Discharge Planning assists with LTC, SNF, Rehabilitation, Behavioral/Mental Health, Home Care, DME, transportation and other placement planning needs.
  • Discharge Planning assists with interactions with CPS, APS, Adoptions and end of life planning and completion of Advance Directives.
  • Discharge Planning is a liaison with third-party payers and government programs to access special services for patients.

Minimum Qualifications:

  • ADN or BSN required.
  • Current licensure applicable to professional affiliation required.
  • Certification by the National Association of Healthcare Professionals or InterQual is desirable.
  • Current New Mexico licensure as a Registered Nurse.
  • Certification as CCM or ACM desired.
