What are the responsibilities and job description for the Care Coordinator Hospice position at Memorial White Rose Hospice?
The Care Coordinator is responsible for managing patient referrals and visit schedules and supporting the hospice
clinical team. Assigns patient assessments and other visits, as ordered by the physician, using the online
scheduling system. Collaborates with the Patient Care Manager to identify clinicians with the appropriate
experience and skill set to match patient needs.
- Utilizes an automated scheduling system to maintain a calendar of services for patients.
- Processes workflow for requested scheduled, missed, rescheduled, reassigned, declined, and/or delivered visits.
- Helps to manage order tracking, IDG preparation, and other tasks associated with the management of patient care.
- Monitors pending referrals daily and assigns registered nurse or advanced practice nurse for all start of care visits.
- Demonstrates appropriate communication and follow-up when answering phone questions from patients, families, referral sources, staff, and other community resources.
- Reviews Unverified Visit report daily. Communicates daily with field staff and/or clinical lead to resolve any visits in late, pending, unverified, or incomplete status.
- Runs Scheduling Request report daily. Completes scheduling requests according to physician orders for visit frequencies during the Medicare week.
- Monitors productivity and distribution of visits and communicates any needed changes with the clinical leader, as appropriate.
- Ensures that the patient visit schedule supports continuity of care and excellence in patient care management by scheduling most nursing visits to the patient’s designated RN Case Manager, Social Worker,
- Chaplain/Spiritual Counselor, and Aide.
- Participates in patient care team meetings to assist with facilitating timely care, via the scheduling process, to patients suffering from symptom crises, transitioning to end of life, or who are experiencing other immediate physical, psychosocial, or spiritual care needs.
- Continually monitors assigned workflow and processes all tasks, coordination notes, and administrative tasks in a timely manner.
- Assists with coordination, scheduling, and tracking of all visits to ensure compliance with all hospice policies and state and federal regulatory requirements.
- Maintains annual competencies and participates in in-service programs as assigned

Education Requirements
- High school education or equivalent experience requirements
Experience Requirements
- Minimum one year of scheduling experience in health care setting using an online scheduling system.
Skill Requirements
- Exceptional organizational, customer service, communication, and decision-making skills required.
- Working knowledge of state and federal regulations governing the hospice Interdisciplinary Group (IDG) requirements, Aide and/or LPN/LVN supervisory visit requirements, and assessment visit requirements.
- Follows the approved plan of care frequency.
