Job Purpose
The Care Coordinator (CH) serves as a front-line liaison and advocate for patients, helping to connect them to the care they need. This position provides individualized, needs-driven, patient-family centered, case management services to patients and their families, including connecting the patient to community-based organizations and public services that improve health and well-being. The position plays an integral role in maintaining the integrity of the program, and will require some interaction with Spanish-speaking patients as well as external stakeholder and medical office personnel. Spanish proficiency is highly recommended and preferred, but is not required. The project’s primary mission is to increase access to specialty care for West TN’s uninsured population.

This specific position will be based at Church Health within Crosstown Concourse.

Duties and Responsibilities

Case Management

  • Work as a team to coordinate patient care, communicating updates and follow-up instructions with patients along the way.
  • Track, manage, process referrals from physicians, community-based organizations, and healthcare administrators.
  • Receive faxes and emails / discuss medical issues and potential eligibility with clinic provider.
  • Determine eligibility for re-enrollment patients.
  • Collect financial records from patients.
  • Document any barriers to eligibility.
  • Communicate with patient, via phone and written correspondence.
  • Communicate with primary care home and specialist via phone and written correspondence.
  • Enter captured information into electronic medical records.
  • Provide ongoing support until patient is discharge.
  • Patient Advocacy (Helping English and Spanish-speaking patients navigate the health care systems and locate additional resources beyond the scope of our program when needed). This may include coordinating transportation, telehealth consults, and providing information regarding the insurance marketplace and other social services such as interpretation.
  • Assist patients with administrative requirements for seeking care with physician offices, surgery centers, and hospitals.

Teamwork and Daily Expectations

  • Work closely with a team of coordinators at Church Health and internal stakeholders to accomplish daily tasks.
  • Directly responsible for managing referrals received from clinics, with precise attention to detail, process, and follow-through.
  • Maintains positive relationships with patients and external program partners.
  • Accurate and timely data entry and record maintenance.
  • All other duties as assigned.

Qualifications include:

  • High School diploma required. Four-year degree preferred. Current undergrad or grad students near graduation will be considered.
  • Strong attention to detail is required as well as exceptional interpersonal and relational skills.
  • One year of healthcare administration experience preferred.
  • Fluency, both written and verbal, in Spanish preferred.
  • Must be a team player and demonstrate ability to quickly learn and adapt new skills.
  • Exceptional multi-tasking skills are critical for success in this role.
  • Excellent customer service and organizational skills.
  • Proven ability to communicate both in written and verbal forms.
  • Proficiency in the Microsoft Office suite of computer programs (Word, Excel, etc.)

Working conditions:

This position is grant-funded. This job requires some local and regional travel. This job also requires some night and weekend activities. Otherwise, work is typically conducted between 8am – 5pm Monday-Friday.

Benefits: The grant covers office equipment and healthcare benefits and will provide resources for medical, vision, and other insurance policies

Physical requirements:

Ability to lift up to 40 pounds on an as-needed basis.

Job Type: Full-time

Pay: From $41,000.00 per year
