Position Title: Case Manager/Record Coordinator
Reports to: Residential Manager
Essential Functions /Job Duties:
· Scheduling appointments/meeting/reviews
· Answering business phone
· Utilizing and maintaining state website systems (monthly reviews, service plan reviews)
· Logging daily records into the system once a week
· Maintaining employee files for state licensing requirements, including employee training records
· Maintaining constant contact with county workers/ guardians
· Attending scheduled trainings
· Adhering to ethical and professional standards
· Scheduling annual and follow-up appointments for residents
· Transporting residents to appointments, including but not limited to, dental, vision, & medical
· Communicating with medical dept. regarding medication needs. (Request refills, counting, tracking medication)
· Overseeing resident medication management, including changes in medication documentation on MARS.
· Maintaining residential youth files to standard required
· Working in a team context to maintain necessary documentation to meet compliance requirements
· Performing other duties as assigned
The successful applicant must demonstrate the ability to adhere to ethical and professional standards, as well as be viewed as a: good communicator, multi-tasking, adaptable to working as a team and on their own for task.
Minimum Requirements:
· High school diploma or GED
· Work well with computers and be able to use MS Excel/Word/email/etc.
· Must have vehicle, driver’s license, and car insurance
· Must be 21 years of age
· Must acquire a successful background screen including FBI/BCII
· Must provide a successful drug screen
· Successful health screening
· Produce a clean abstract driving record
· Able to effectively communicate through verbal/written expression
· Preferably possess a Bachelor degree or other related degree in the human service area or medical
· Preferably one-year of related experience working with children and adolescents
