MCHC Health Center in Ukiah, CA is seeking a full-time Health Information Management (HIM) Clerk to join our dedicated team. In this important role, you'll help support compassionate, high-quality care for our communities by collecting, processing, and maintaining patient health information in both electronic and paper formats. You’ll ensure that accurate and timely data is readily available to authorized users, playing a key part in delivering the best possible care when it’s needed most.
Duties Include :
- Demonstrates adequate knowledge of medical terminology, medical procedures, medications, immunizations, and ICD-10 codes.
- Demonstrates Proficiency in daily chart tasks such as dictation processing, referral scans, and ROI abstraction.
- Processes and documents requests from ancillary departments and release of information (ROI) requests and incoming records through the electronic health record (EHR) and paper chart; copies medical records and demonstrates proficiency in accurate and timely data entry.
- Processes transcribed provider dictation.
- Merges duplicate medical record numbers electronically in the EHR and in general data entry.
- Processes patient health information for delivery, retrieval, reconciliation, filing and destruction of paper–based and electronic records both alphabetically and in terminal digit.
- Attends education / training for patient privacy and HIPAA compliance. Participates and promotes activities to support department compliance with Personal Health Information, PHI, information privacy awareness within HIM and MCHC to minimize the risk of PHI breaches.
