Personal Service Coordinator
35 hours per week / $24.00 per hour
(The actual compensation will be determined based on experience and other factors permitted by law.)
Summary: MHALA is partnering with Dignity Health, to provide targeted outreach and case management services to individuals who frequently visit the St. Mary’s Hospital Emergency Room, and who also may be experiencing challenges such as mental illness, homelessness, substance use, or difficulties accessing healthcare. The Personal Service Coordinator will provide intensive community-based case management services based off of an individual recovery plan, to connect individuals with needed resources in the community with the overall goal of improving their quality of life and reducing the frequency of visits to the emergency room.
Essential Duties and Responsibilities: This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director. - Outreach, engage, and build relationships with individuals referred to the HealthLink program.
- In collaboration with the MSW, assess the needs of individuals referred, and provide community-based case management services, linking individuals with needed resources including but not limited to linkage to emergency/transitional housing, mental health services, insurance, healthcare providers, benefits, substance abuse treatment, and any other needed resource.
- Provide brief solution-focused linkage, support, coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such physical health care, money management, grooming and hygiene, meal preparation, public transportation and other independent living skills
- Mediate and advocate on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements, etc.
- Utilize evidence based practices in service delivery such as motivational interviewing, harm reduction, intensive case management, housing first, and critical time intervention
- Link individuals with emergency financial assistance as appropriate to provide emergency housing, food, clothing, transportation, etc.
- Work collaboratively with St. Mary’s staff to facilitate referrals to HealthLink program, participate in collaborative meetings with St. Mary’s, and provide active feedback on development of program.
- Provide timely information and feedback to supervisor with regard to emergent participant and program issues.
- Build collaborative partnerships with existing and new community resources and providers for linkage and support purposes.
- Attend and participate in staff meetings and other aspects of the day-to-day operations and community life of the program site(s)
- Complete all documentation and maintain all paperwork and other required documentation as required for program compliance and accountability in accordance with MHALA standards and funding source requirements.
- Bilingual in English/Spanish preferred but not required.
- Knowledgeable in technological and computer applications such as Microsoft Word, Excel and Outlook
- Demonstrate knowledge of community resources
- Must function well in a team-oriented and fast-paced environment to member services
- Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHALA’s liability insurance are conditions of employment
- TB clearance and re-testing every year are conditions of continued employment
