Medication Room Nurse

35 hours per week / $34.00 - $36.00 per hour

(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: Under general supervision of Senior Clinical Director of Adult Team Services, serves as a support to teams and mentally ill adults by providing on-site medication delivery services. May provide license-dependent physical and mental health services as needed.

Essential Duties and Responsibilities: This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director. • Dispenses medication to members according to physician prescription and in keeping with funding source requirements and professional standards and practices.
• Provides day-to-day oversight of medication room, including daily management of administrative tasks such as logs and other records, charting of services performed, compliance with protective technical polices and procedures, and other similar duties.
• Demonstrate responsibility for the proper transcribing, administration, monitoring, and managing of medications. Display knowledge of adverse side effects, indications for use, and contraindications of medications
• Assist in the collection of health information. Monitor and manage health conditions by taking measurements of weight/BMI, blood pressure, glucose levels, and lipid profiles
• Develop collaborative relationship with psychiatrists, nurse practitioners, physical health care providers and interdisciplinary personnel to address complex needs of individual members
• Provide education to members regarding health promotion, medications, and disease management. Focus on skill building to enhance member’s ability to handle health concerns independently
• Educate Personal Service Coordinators and aid with medication assessments
• May provide information, coaching, counseling and training to members in the area of medication management, and/or provide other physical or mental health services to members in keeping with scope of license and practice.
• Participates in staff and community meetings and in the day-to-day operations of the program as needed.
• Maintain all records and documentation in accordance with MHALA, County Contracted Provider, Welligent system (EHR) and funding source standards and requirements, and documents in required program, including 24 hour turnaround documentation
  
  

Position Requirements

• Licensed Vocational Nurse/ Licensed Psychiatric Nurse (LVN/ LPT) license
• Minimum 3 years of experience providing psychosocial rehabilitation related services to a disabled population preferred
• Demonstrate knowledge of community resources
• Must function well in a team-oriented approach to member services
• Knowledgeable in computer applications such as Microsoft Word, Excel and Outlook
• Flexible work schedule to include some evenings or weekends, and on call cell phone services when necessary
• Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHALA liability insurance are conditions of employment
• TB clearance and re-testing every year are conditions of continued employment
