Summary: On a daily basis, counselors will work with the Program Director to provide training, support and assistance to the program participants. Counselors will ensure safe, supportive and home-like environments in which residents may learn and/or improve the skills necessary to transition to a higher level of independent living.

Essential Duties and Responsibilities include the following:

1. Train, assist, monitor and support residents in activities of daily living.

2. Develop and implement individual service plans and monitor programs.

3. Provide informal, situational counseling with a focus on teaching residents to improve and develop decision-making, problem-solving and coping skills

4. Ensure the physical safety of the residents.

5. Be responsible for training and assisting residents in proper self-administration and storage of medications. Monitor and record prescriptions, refills and medication counts to ensure medication compliance.

6. Assist in facilitating case management services as initiated by the Apartment Program Director.

7. Attend weekly staff meeting and participate in the team process.

8. Focus on preventative approaches in areas of behavioral intervention.

9. Complete necessary recordkeeping and documentation in a timely manner.

10. Respond to emergencies, crises or acute episodes of emotional distress of residents.

11. Act as an effective role model.

12. Encourage residents to take part in planned day programming and monitor follow-through.

13. Be responsible for on-call duties, specifically, carry the crisis phone on a weekly rotating basis.

14. Facilitate, assist in planning and implementing recreational activities.

15. Monitor and ensure the safety and cleanliness of apartment sites.

16. Ensure residents’ medical needs are met and that annual physical and dental exams are scheduled.

17. Use personal vehicle to provide transportation services to residents as needed.

18. Implement and document monthly fire drills.

19. Participate in appropriate, available trainings, professional developments and in-services.

20. Participate in new resident orientation.

21. Be involved in follow-up services, assist with moving, and facilitate and support the transition between program levels or to independent community living.

22. Complete discharge planning meetings on a semi-annual basis.

23. Comply with all OMH regulations for residential services.

24. Participate in agency events.

25. Adhere to all NYS motor vehicle traffic laws.

26. Other duties as assigned.

Requirements include excellent oral and written communication skills; understand and accept the Mental Health Association’s Personnel Policies and Procedures and Code of Conduct; ability to deal calmly and competently with several demands at one time; have your own transportation and a clean license. Possible use of personal vehicle for work related activities. A criminal history review, DMV clearance and NYS Central Register clearance is necessary. Adherence to the Justice Center for the Protection of People with Special Needs regulations and requirements including annually signing and continuously following the Center’s Code of Conduct and Mandated Reporter policies. Adhere to the Association’s Medicaid Compliance Plan that outlines the importance of each employee’s responsibility to ensure there is no intentional waste, fraud, or abuse of taxpayer’s dollars.

Other Skills and Abilities that are essential to this position include strong interpersonal, communication and organizational skills. An employee must be dependable, flexible, and be able to work independently and as part of a team.

Job Type: Full-time

Pay: $16.00 - $18.00 per hour

Expected hours: 37.5 per week
