Title : Senior Care Manager- Position located in Middletown
Reports to : Care Coordination Program Manager
Job Objectives :
Provide care management to individuals enrolled in the Health Home Care Coordination Program, while also mentoring other care managers within the department.
Perform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange.
Responsibilities :
- Attend bi-weekly Care Coordination team meetings.
- Attend trainings and meetings relevant to Health Homes.
- Deliver care coordination services as outlined under Health Home Model for Care Coordination for a caseload of between 20-40 program participants.
- Complete New York State Community Mental Health (CMH) assessments and reassessments.
- Complete monthly scorecard data collection information.
- Maintain all client records electronically through Foothold Technology and FCM Electronic Record Keeping.
- Maintain all client files according to program policies and procedures.
- Establish and maintain linkage with appropriate community agencies and service providers.
- Represent MHA at meetings as necessary.
- Assist with internal audits of participant charts.
- Provide oversight to other care managers in the field, as needed.
- Assist staff when program participants may be experiencing a crisis and / or responding to phone calls from participants in crisis.
- Answering general care management and health home questions for staff and ensuring a manager or supervisor is informed if the question or situation is more complex.
- Assisting with training and / or shadowing new care management staff.
- Reviewing time management strategies and deadlines with staff, as needed.
