MHA is hiring immediately for a Clinical Case Manager. The Clinical Case Manager in the Young Adult Supported Community Living program will work with young adults aged 18 – 22 with a case load of no more than 10. The Clinical Case Manager will work with supervisory and program staff within the young adult supported community living program to develop Adult Comprehensive Assessments, Risk Assessments, CANS, All ACA addenda, FCR, and Treatment plans that meet the standards established by DCF regulations. Minimize barriers in an effort to achieve the delivery of high quality evidence-based practices, programs and services and ensure overall clinical management of programs in accordance with executive leadership objectives, organizational mission, policies, practices, program funding and applicable regulatory agency guidelines.
Position’s Essential Functions
- In consultation with supervisory personnel, develop comprehensive assessments and treatment plans which capitalize on the strengths and resiliencies of each young adult while addressing functional limitations and barriers to independence.
- Provide consultation, assessment and treatment planning in the development of pre-crisis and crisis plans, behavioral guidelines, risk assessments, positive behavior plans, medication treatment plans, etc.
- Complete thorough reviews of historical documents as needed to ensure a clear understanding of relevant history. Consult with program staff, family, and other collaterals as necessary to develop clear, concise, user-friendly treatment plans / interventions and data collection system.
- Under the direction of the Program Director, train all necessary staff in behavioral and other clinical interventions including data collection procedures.
- Participate in program related meetings both inter / intra agency including team meetings, risk meetings, IAP meetings, medication reviews, etc. Keep abreast of the progress and/or barriers of established plans.
- Work collaboratively with teams/programs to develop and implement individualized action plans.
- Work collaboratively with teams/programs to develop and implement program initiatives.
- Obtain and maintain CANS certification.
- Serve as a liaison to the DCF service system, state agencies, families, and community partners as needed. Create and maintain open and effective lines of communication in an effort to meet contract standards and promote the mission of MHA within the community. Establish and maintain relationships with local, state, federal, and private agencies and organizations to promote programs and to identify and secure potential new alliances, partnerships and funding sources.
- Create and facilitate psychoeducational and skill-building groups relevant to the needs of each program. Model and teach group facilitation skills to direct-care staff.
Education and Professional Experience:
- Master’s Degree in a Social Work, Psychology, Mental Health Counseling or related field.
- License Preferred/License Eligible considered (at least one of the following): LMHC, LCSW, LMFT, LICSW, LADC 1.
- Minimum of 3 years of experience working with individuals with psychiatric disabilities, challenging behaviors, trauma and/or mental health issues in a clinical position.
