Position Summary:
The Projects for Assistance in Transition from Homelessness (PATH) Case Manager plays a crucial role in supporting individuals who are homeless or at imminent risk of homelessness and experiencing serious mental illness. This position is responsible for providing comprehensive case management services, linking consumers to vital community resources, and ensuring their successful transition toward stability and independence.
Reports to: PATH Program Director
Case Management and Client Support:
- Provide comprehensive case management for assigned consumers, ensuring their needs are met through a person-centered approach.
- Conduct outreach to individuals who are homeless or at risk of homelessness, establishing trust and building positive rapport.
- Assist consumers in accessing and utilizing all available community resources, including housing, healthcare, and social services.
- Assess consumer needs, develop individualized service plans, and link them to appropriate community supports.
- Complete all required documentation in consumer files and the New Jersey Homeless Management Information System (HMIS) in compliance with policies and regulations.
- Maintain updated knowledge of each assigned consumer’s situation, including key contacts and support systems.
- Develop crisis plans with consumers that incorporate available 24-hour community services.
- Educate consumers on mental health, substance use, and crisis management strategies.
Collaboration & Professional Development:
- Engage with community partners, service providers, and stakeholders to coordinate resources and enhance service delivery.
- Attend agency staff meetings, peer supervision meetings, and community collaboration meetings to support program objectives.
- Participate in ongoing professional development to stay informed on best practices in homelessness intervention and mental health services.
Required Skills & Competencies:
- Strong clinical assessment skills with the ability to interpret consumer charts and recognize behavioral health needs.
- Ability to identify and respond to clinical crises that require emergency intervention.
- Experience conducting bio-psychosocial assessments and developing individualized service plans.
- Cultural competency and ability to engage consumers with diverse backgrounds and experiences.
- Strong networking skills with the ability to connect consumers to both formal and informal community resources.
- Knowledge of mental health conditions, substance use disorders, and their impact on overall well-being.
- Excellent written and verbal communication skills, with the ability to document case progress accurately.
- Safe driving habits, as demonstrated by a clean motor vehicle record.
Education & Experience Requirements:
Bachelor’s degree in mental health, social work, psychology, or a related human services field.
Minimum of 2 years of relevant work experience in case management, homelessness services, or mental health support.
