The Case Manager II Housing Services is responsible for providing advanced case management services to individuals with complex mental health conditions, substance use disorders, and other social challenges. This role entails managing a more complex caseload, offering higher-level advocacy, and often leading coordination efforts among interdisciplinary teams and external providers. 

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Conduct thorough assessments of clients' mental health, physical health, housing, and social support needs.
  • Develop individualized service plans based on client goals, ensuring plans are updated regularly to reflect progress or changing needs.
  • Assist clients in navigating systems of care, including housing, healthcare, employment, and social services.
  • Collaborate with internal programs and external partners to connect clients with necessary resources.
  • Advocate on behalf of clients to ensure access to quality services and fair treatment.
  • Empower clients to self-advocate and make informed decisions about their care.
  • Maintain regular contact with clients through home visits, phone calls, and office meetings to monitor progress and address concerns.
  • Provide crisis intervention and emotional support as needed.
  • Accurately document client interactions, assessments, service plans, and outcomes in accordance with agency policies and regulatory requirements.
  • Ensure all client records are kept confidential and in compliance with HIPAA.
  • Work closely with the clinical team, including therapists, psychiatrists, and peer support specialists, to provide integrated care.
  • Participate in team meetings and case consultations.
  • Attend ongoing training and workshops to stay updated on best practices in case management and mental health care.
  • Maintain any required certifications or licenses for the position.


KNOWLEDGE, SKILLS & ABILITIES:

  • Bachelor’s degree in human relations/behavioral health/social work/medical health related field and/or extensive experience in case management working with vulnerable populations representative of various forms of diversity, disability, and complex needs is referred. 
  • Prior experience in case management, mental health, or social services is preferred.
  • PRSS Certification preferred. If PRSS Certified, agree to respect and observe the PRSS Code of Ethics.
  • Strong interpersonal and communication skills.
  • Ability to work effectively with diverse populations and handle sensitive situations with empathy and professionalism.
  • Be stabilized in recovery and practice ongoing self-care.
  • Be able to serve as an example for others of what wellness and recovery looks like.
  • Excellent organizational and time-management skills.
  • Proficiency in Microsoft Office and Outlook. Salesforce experience is preferred.
  • Must possess a valid driver's license and have reliable transportation.
  • CPR/First Aid certification or willingness to obtain upon hire.

 

WORK CONDITIONS & PHYSICAL DEMANDS:

  • Work is performed in climate-controlled office setting, where exposure to conditions of extreme heat/cold, poor ventilation, fumes and gases is unlikely. 
  • Noise level is moderate and includes sounds of normal office equipment (printers, fax machine, phones, etc.)
  • No environmental hazards are encountered in normal performance of job duties. 
  • Ability to transport participants as needed to improve independence, quality of life and achievement of self-sufficiency.
  • Work requires a flexible work schedule due to internal and external meetings. Occasional travel may be required; must have reliable transportation and a valid Oklahoma Driver’s license. 
  • Ability to effectively communicate orally and in writing.
  • Physical ability to move 25 pounds on occasion.
  • Visual and auditory ability to identify and respond to environmental and other hazards of the site and staff behavior.
  • Physical mobility and endurance to perform tasks while standing/walking for long periods of time (60 minutes or more).
  • Ability to safely and properly use office equipment.
  • Ability to climb stairs.
