POSITION TITLE:              Intensive Case Manager


REPORTS TO:                     OCI Coordinator


POSITION OVERVIEW:   The Intensive Case Manager provides intensive and short term case management to high risk behavioral health consumers with the overall objective of securing and maintaining housing, and access and engagement to mental health services. The Case Manager will assist the consumers to improve their independent living skills and manage the symptoms of their mental illness. The Intensive Case Manager is responsible for the assessment and collaborative development of the consumer’s goals and aspirations and coordination of services to meet their goals.  

 

ESSENTIAL POSITION RESPONSIBILITIES:

1.   Makes an impact on at least 5 consumer’s per day through face-to-face interventions. 

2.   Meets deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and electronic timesheets.  

3.   Maintains accurate and timely documentation of service provision. Completes progress notes daily in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.

4.   Assists clients in setting recovery goals in utilizing the strengths assessment process and update quarterly.

5.   Completes treatment plan using goals identified from strength assessment and update quarterly.

6.   Completes DLA 20’s as needed in a timely manner. 

7.   Employs recovery strategies in working with consumer using the Housing First and Strengths Based approach.

8.   Assists consumers to achieve housing/educational/employment goals and independent living.

9.   Ensures consumers are able to meet ADL’s. 

10.  Meets with consumers to assist and provide instruction necessary to meet identified goals and work toward recovery and reintegration.

11.   Monitors status of client and provides level of personal and other supports consistent with consumer status. Notifies supervisor of change in client status.

12.   Coordinates other services required to meet identified goals based on the needs of the clients.

13.   Monitors progress of other services on an ongoing basis to evaluate effectiveness of service  and providing revision as necessary with documentation on progress notes.

14.    Assists consumers to access/maintain all available entitlements including SSI, SSDI, food stamps, medical cards and others.

15.    Assists consumers with crisis situations and/or in developing a crisis plan.

16.    Attends and participates in individual, group and field supervision, as established by the program.

 

OTHER POSITION REQUIREMENTS:

1.         Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required.  Ensures appropriate notification to supervisor for absences, and ensures that work is covered.  Flexible in work schedule when needed.

2.         Exhibits appropriate level of technical knowledge for the position.  

3.         Produces quantity of work necessary to meet job requirements.  

4.         Works well with a team, keeps others informed of information needed.Treats others with respect, maintaining a spirit of cooperation.

5.         Maintains effective and professional verbal and written interactions with peers, customers, supervisors and other staff.  Uses diplomacy and tact in dealing with difficult situations or people. Demonstrates effective listening skills. Is receptive to constructive  feedback.

6.         Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identifies what needs to be done and takes  appropriate action.   

7.         Completes assigned    work, meets deadlines without reminders/follow-up from supervisor or others.  

8.         Performs work conscientiously with a high degree of accuracy.  

9.         Meets goals and objectives as mutually agreed upon during last performance review (if applicable).

POSITION REQUIREMENTS:  The Intensive Case Manager is expected to have a Bachelors degree in a human service field or four years of equivalent education and/or experience working in the human services field.  Computer literacy required.  Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working in the community with individuals who have serious and persistent mental illness; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with clients, constituents and staff.  A valid Kansas driver’s license and access to personal vehicle required.   

PHYSICAL REQUIREMENTS:

*          Driving (for purposes of community mobility)

*          Typing/data entry, writing

*          Lifting/carrying up to 30 pounds

*          Bending/Stooping/Climbing stairs
