- Schedule CNA visits and report changes in patient conditions.
- Coordinate care with interdisciplinary team members.
- Ensure timely signing of telephone orders and track medical supplies.
- Participate in quality improvement activities and maintain professional development.
- Graduate of an accredited nursing school with 1-2 years of acute care and home care experience.
- Current nursing license and CPR certification required.
- Excellent communication and nursing skills.
