CARE MANAGER - LOWER HUDSON VALLEY (BILINGUAL IN SPANISH PREFERRED)

Position Summary: The Care Manager oversees and coordinates access to services for individuals with intellectual and developmental disabilities, developing and monitoring person-centered Life Plans. Responsibilities include conducting assessments, advocating for members, and ensuring access to healthcare and community services.

Duties & Responsibilities

• Deliver person-centered care management services.
• Complete comprehensive assessments and develop Life Plans.
• Advocate for members' needs and access to services.
• Maintain eligibility for care management and benefits.
• Coordinate healthcare services and transitional care.
• Utilize health information technology for service delivery.
• Travel to meet members and providers as needed.
  
  

Qualifications: Bachelors degree or equivalent experience, strong communication skills, and ability to work autonomously. Remote position with travel required.

Job Type: Full-time, Pay: $29.00 - $31.00 per hour.
