Must possess a valid NJ Driver's License.ResponsibilitiesProvides community based outreach services to persons diagnosed with a psychiatric disorder. Performs case management duties including linkage and referrals to mental health and non-mental health services, service coordination, and assists clients with developing service plans.Services will be delivered in the individual's home or other locations convenient for the individual.
