What are the responsibilities and job description for the Case Manager position at Mental Health Association?
Qualifications: Bachelor's degree in a human services field. Must have at least one year of related experience. Computer literacy and a Valid driver’s license and the ability to drive a passenger van is required.
Responsibilities:
- Responsible for the daily implementation of the Partial Care Program, in collaboration with staff, supervisors and members, to provide partial care services to individuals with severe and persistent mental illness, in order to enhance community functioning.
- Plan and facilitate milieu activities, including therapy, coping, activity and psycho-educational groups to enhance social skills and promote relapse prevention and independent living.
- Collaborate with members and staff to accomplish psycho-social goals and tasks of the unit.
- Provide milieu management.
- Participate in unit trips including van driving
- Provide case management services for members as assigned, including coordination of medical, psychiatric, financial, residential, social and legal needs of assigned caseload.
- Regular goal-setting with members.
- Crisis intervention, as needed
- Assist and advocate for members to obtain medical records, entitlements, especially Medicaid/Medicare and food stamps.
- Liaison with family, boarding home staff and local agencies which serve the elderly.
- Consult regularly with Prospect House program psychiatrists.
- Arrange transportation services and provide transportation as needed.
- Collaborate with the Program Director and Supervisor to develop the program, expand services, select supplies and materials and research new program methods and activities.
- Complete in-service training as required.
- Must meet all expected productivity levels in regard to direct service provision to consumers.
