We are a leading provider of innovative medical care for long-term and post-acute patients in the U.S. We are seeking a full-time Nurse Practitioner or Physician Assistant to provide on-site medical care in a Skilled Nursing Facility in Oswego, NY.

Responsibilities Include

  • Conducting health assessments and developing care plans
  • Managing chronic conditions and acute illnesses
  • Ordering diagnostic tests and prescribing medications
  • Educating residents and families on health maintenance
  • Collaborating with healthcare professionals for coordinated care
  • Maintaining accurate documentation

Requirements: Current NP licensure in NY, board certification, experience with geriatrics preferred, strong communication skills, and proficiency in electronic medical records.
