Summary
The ACT RN Case Manager plays a vital role within a multidisciplinary team, delivering intensive, recovery-focused services to individuals with severe and persistent mental illness who are at high risk of unsuccessful independent living and require institutional care, services, and treatment. This position takes a strengths-based approach to care, overseeing the medication system, documenting treatment, and coordinating with community medical providers. Additionally, the RN Case Manager provides health education, promotion, and training to team members while supporting clients in understanding their prescribed medications and the importance of adherence to treatment.
Essential Functions

- Attends ACT team meetings to review referrals, address issues and report on client activities and progress.
- Provides direct nursing services to adults through ACT model.
- Assesses client health and responds appropriately to any noted concerns (i.e., medication side effects, withdrawal symptoms, general health changes, etc.).
- Monitors client use of medications, changes in medications, and assesses for efficacy, safety, and/or side effects of medications.
- Collaborates with all providers involved with client’s health care and provides feedback to providers as needed.
- Links client to appropriate healthcare providers or specialists and follows-up with all referrals and recommendations.
- Provides health and wellness education to the client, the client’s supports, and other ACT Team staff members.
- Participates in the formal training and development process with administrative staff and consultants as well as ongoing professional development and continuing education.
- Attends daily team meetings, individual supervision, and group supervision as scheduled.
- Initiates and maintains relationships with community resources, outside human service agencies, and other stakeholders as needed.
- Ensures fidelity to the ACT model and is committed to ongoing quality improvement efforts.
- Completes all required documentation accurately and on time.
Work Environment
This job operates in the community, client homes, and a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

Requirements:Skills & Abilities Education & Experience
- Bachelor’s degree from an accredited school of nursing is required.
- Minimum 1-year of experience in acute care nursing (Med Surg, ER, ICU, Psych).
- Minimum 1-year of experience working with Severe and Persistently Mentally Ill (SPMI) individuals.
Certifications & Licenses
- Licensed as a Registered Nurse (RN) by the Kansas State Board of Nursing to practice without limitations.
- Must complete Case Management Training and provide certificate within 6 months of hire. Complete training in one or more specialized service.
Computer Skills Computer experience in Microsoft Office programs and basic computer fluency. Additional Qualifications Knowledge, understanding, and experience providing mental health and substance use treatment, including experience conducting mental health evaluations, familiarity with psychiatric medications, and awareness of medication assisted treatment (MAT) practices; ability to respond to client needs using a trauma-informed, recovery-oriented approach; comfortable working with clients diagnosed with severe and persistent mental illness in a variety of office, home, and community-based settings; ability to function as an integral part of the collective treatment team; knowledge of local, state and federal regulations, laws and standards with regard to behavioral health treatment services; strong oral and written communication skills and ability to communicate effectively with clients, family members, colleagues, and professional contacts; ability to plan and prioritize work and meet CrossWinds’ performance, quality assurance and productivity standards.
