What are the responsibilities and job description for the Case Manager/ Low Intensity Care Coordinator position at Mental Health Center of North Central Alabama?
DUTIES AND RESPONSIBILITIES:
- Needs assessment. (Example of tasks: using required forms; determines through assessment client's assets, deficits and basic needs; reviews needs on regular basis).
- Develops service plan. (Examples of tasks: contact provider source; write goals which may include personal grooming and health teaching; medication knowledge; financial aid; etc.).
- Implements plan by providing case management services. (Examples of tasks: monitor client condition; make home visits; assist client in securing needed services).
- Basic Living Skills Instructor. (Examples of tasks: provides didactic, process and experiential instruction to clients).
Job Type: Full-time
Pay: $14.66 - $15.87 per hour
Expected hours: No less than 40 per week
