Job Title: Community Health Worker
JOB SUMMARY: Provides community based care to Consumers and serve as link between community and health/social services to expand access to care and improve health outcomes.
RESPONSIBILITIES:
- Work within team environment agency-wide and in partnership with all departments to serve Consumers.
- Meet regularly with Consumers to assist them with achieving progress towards their goals.
- Assist Consumers with connecting to treatment and resources to address needs regarding mental health, physical health, substance use, and social determinants of health.
- Provide education to Consumers to help in understanding appropriate use of ER, Urgent Care, and PCP.
- Assist Consumers with understanding their gaps in care and developing action plans to close gaps.
- Identify and share with staff and Consumers community resources that foster improved health and well being.
- Transport and/or accompany Consumers to appointments as needed. Assist with scheduling transportation as needed.
- Coordinate care between Consumer supports and providers.
- Document in electronic health record all efforts to engage Consumers and assist in their care.
- Attend team meeting daily and come prepared to actively participate.
- Collaborate with team members and other departments.
- Utilize the IMPaCT care model in work with Consumers.
- Attend all necessary trainings, meetings, and supervisions as scheduled.
REQUIREMENTS:
- High School Diploma/ GED
- Valid Tennessee Driver’s License
- Acceptable Motor Vehicle Report (MVR)
- Acceptable Criminal Background Investigation
