Position Details
The Behavioral Health Urgent Care Case Manager will provides immediate and follow-up case management services to individuals experiencing urgent behavioral health challenges. Primary responsibility will be to offer comprehensive support, including benefit enrollment, information about external resources including addressing the Social Determinants of Health, referrals, and completing outreach contact calls. Will require a collaborative and empathetic approach to help individuals access the resources necessary to address their health needs and improve overall well-being.

The Ideal Candidate Will Possess The Following Qualifications
- Bachelor’s degree in social work, Psychology, Counseling, or a related field is required.
- Knowledge of public and private benefit programs and community resources related to Social Determinants of Health (SDOH) is required.
- Excellent communication and interpersonal skills to engage effectively with individuals and colleagues and computer competent.
- Bilingual candidate in Spanish and English is preferred.
- Previous experience in case management, social work, or related roles, preferably in behavioral health or crisis settings is preferred.
