We are seeking a Case Manager for our Fair Assessment Program to join our team in our Wilmington, DE area.
Position Details
* Coordinates resources and provides information tailored to client needs.
* Efficiently documents all resource coordination activities.
* Coordinates referrals for client-specific discharge planning, ensuring active family participation in creating a comprehensive discharge plan that reflects their choice of service providers and community resources.
* Monitors record-keeping, compliance, and remaining authorized units, ensuring client charts are current and include necessary documentation, such as incident reports within 24 hours when required.
* Assesses, monitors, and documents safety concerns.
* Completes case documentation in compliance with standards set by Merakey, county MHDS/CASSP, OMHSAS, and other relevant licensing, regulatory, and payor organizations.
Earn: $20.67 per hr.

The ideal candidate will possess the following qualifications:
* Bachelor's Degree required, preference is in psychology, sociology, social work.
* 1-3 years of experience working with children.
* Valid Driver's license and working vehicle
