Position Details:
- Under the direction of the Blended Case Management (BCM) Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan.
- This position will be based home but will have 12-15 youth and young adults (15-26 years of age) per week
- Case load will be located in Cambria County (mileage will be paid)
- As a member of the BCM team this position will serve as primary contact for youth and young adults.
- Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients' needs along with supportive counseling.
- Will include on-call responsibilities; rotation approx. 1 week a month with a $75 stipend

Requirements:Our ideal candidate will possess the following skills:
- Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or, Registered nurse;
- Possess a current PA driver's license with 2 years of driving experience with no suspensions
- A Reliable vehicle (this is a driving position)
- Current car insurance (required)
