Position Details
The transitional coordinator plays a pivotal role in assisting young adults as they navigate the complexities of adulthood, ensuring their mental health diagnosis is managed effectively, while providing them with the necessary tools and support to be successful.
Duties and Responsibilities
- Conduct home visits to work on skill development with consumers and their guardians.
- Provide individual case management services in the community.
- Organize weekly group sessions.
- Educate consumers on independent living skills, employment opportunities, and community involvement.
- Promote physical, emotional, and mental wellness.
- Provide transportation to various appointments and activities.
- Complete thorough documentation of consumer encounters and progress.

Requirements:The ideal candidate will possess the following qualifications:
- Bachelor's degree in a Human Services related field with one year experience working with young adults
- Active and unrestricted driver's license required
- Master's degree preferred
