What are the responsibilities and job description for the Assessment & Training Facilitator position at MERCEDES DIAZ HOMES INC?
At MDH Network, we serve as a source of support and guidance to empower and improve the lives of Individuals with intellectual disabilities, developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement through person-centered planning.
With over 30 residential facilities (“homes”), and several supportive, non-residential programs located throughout Southern California, we strive to support the Individuals live their best lives.
We are seeking a person who is committed to the mental health sector and who is skilled in assessing the needs of Individuals who are interested in receiving our services by determining their preferences, supports needs, and desired outcomes.
Reporting to the assistant program director for our day program in San Diego, this position provides effective support to the service coordination for each Individual new to our services and ensures that reporting requirements for current Individuals are met. They will also conduct the introductory component of the on-the-job training for new staff on program standards, policies, and procedures with a focus on assessment tools, techniques, and best practices.
Candidate Qualifications:
- High school graduate or GED equivalent required
- At least three (3) to five (5) years of relevant experience in a related field, subject, or services
- At least two (2) years working with Individuals that have intellectual, developmental disabilities, or severe behavioral challenges, or dual diagnoses, or forensic involvement required
- Experience working with persons with severe behavioral challenges in behavioral programs preferred
- Experience delivering on-the-job training in a social service environment preferred
- Ability to read, write and speak English. Bilingual in English/Spanish is preferred.

Pay Range: $23.00 - $25.00 per hour depending on experience
