Lead Program Instructor

• A High School diploma or GED equivalent and be at least 18 years old,
• Two years of relevant experience working with Individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or in the care of the medically fragile,
• A current driver’s license and California automobile insurance coverage for driving your personal vehicle to and from various community locations either independently or with other team members and/or Individuals, and if a company vehicle is available for use, one must also be 21 years of age, and 
• Ability to read, write and speak English.  Bilingual in English/Spanish is preferred.

But here’s a known family secret, being part of one, does take effort – it’s all about what you do (your responsibilities/contributions) and how you do it (your behaviors/superpowers) -- so here are some of the things you can expect to experience as you build your FAN club in this house.

There will be chores:  

• As Lead Program Instructor, you will provide assistance with the intake and assessment process of new Program participants and assist in the scheduling of their activities; working with team members to ensure the timely implementation of the Program participant’s Individual Service Plan (ISP) and Restricted Health Care Plan. You will also provide direct support to team members, coordinating work activities in the field and at the site-based facility. You will role model and encourage open lines of communication between team members, the Individuals and the Individual’s family and “circle of support” members (e.g., social service agencies, health care providers, etc.).
• You will assist in the identification, coordination and delivery of formal training and provide just-in-time training to ensure that the Health Plans and Behavior Modification Plans for the Individuals we support are implemented effectively. You will support team members as they perform medical treatment as prescribed and assist with any escalated issues or concerns.   You will role model and ensure all team members are taking special care to document and complete all reporting in an accurate and timely manner.  You will also assist in the review of any special incident reports to analyze and implement corrective actions to prevent reoccurrence and ensure the team and Individuals are safe and secure in their surroundings and during daily activities.

• You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!
• You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).
• You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.
