Shifts:
- Facility-Based Care: day, swing, and night shifts
- In-Home Care: day and swing shifts
- Community Based Care: day shift

*Basic Requirements:
- High school diploma or GED equivalent
- At least 18 years old
- Current Driver's License
- Current Auto Insurance
*some roles may require additional education and/or experience.
Preferred Requirements:
- Bilingual English and/or Mandarin, Cantonese, Russian, Armenian
- Case Management experience
- Experience connecting individuals with generic resources
Experience working with medically fragile individuals
