Job Details

  • A hospital in Arizona is looking for experienced ICU nurse for a travel assignment.
  • The right candidate will have 2 years of experience, and hold an active BLS, ACLS and AZ license or compact.
  • Must pass a dysrhythmia exam upon acceptance (2 max attempts) and there is also a pass / fail Sim Lab on day 1 of orientation.
  • COVID vaccine is required or declination form with supporting documentation from clergy or MD.

  • 2 years of experience required.
  • Active BLS certification required.
  • Active ACLS certification required.
  • Active AZ license or compact required.
  • Must pass a dysrhythmia exam upon acceptance (2 max attempts).
  • Must pass a pass / fail Sim Lab on day 1 of orientation.

    • Schedule Information

  • Every other weekend required.
  • Could be every 3rd weekend.
  • Work into permanent staff rotation for holidays.

    • Unit Specific Information

  • Ratios : 1 : 2 ICU & 1 : 3 MS / Tele
  • Floating : First to Float to tele or the monitor room
  • Floor specific orientation; 2 shifts on the floor.

    • Additional Details

  • Dysrhythmia Exam through UNA is required 24-48hrs prior to signing contract. MAX ATTEMPTS IS 2 - NO EXCEPTIONS, NO 3rd ATTEMPT ALLOWED FOR ANY REASON.
  • If they do not pass in 2 attempts, they will be a DNU for CSH AZ and CSH Sacramento for 6 months.
  • There is a PASS / FAIL Sim Lab on day 1 of orientation. If they fail they will be sent home and made a DNU for 6 months.
