What are the responsibilities and job description for the Resident Care Manager position at Mercy House Ministries?
The Resident Care Manager provides case management services to individuals working toward independence. This includes assessing needs, developing individualized treatment plans, monitoring progress, and linking residents to services and resources. The Resident Care Manager will provide direct outreach to residents in the community and coordinate with their support team. The Resident Care Manager will be a woman of God who operates as an example to each resident we serve. She will pray for/with residents and seek God as she supports our residents.
Key Requirements:
- Must have a public profession of faith in Christ and actively fellowship with a body of believers proclaiming Biblically sound Christian doctrine. Must be submitted to pastoral authority.
- Must have a strong conviction of pro-life values, including the life of the unborn.
- Must have at least an Associate’s Degree or 32 college hours in a related field.
- Must have a clear background check and driving record.
- Must be able to perform work duties virtually.
Responsibilities Include:
Program Development
- Assist Program Director with planning program components to service plan, including but not limited to: Housing, Medical Care, Emotional Care, Life Skills Training
- Research new plan elements for potential additions to the program.
- Regularly review program components with the Executive Director for overall effectiveness and applicability to the needs of residents.
Program Inquiries
- Assist in the initial phone assessment of applicant inquiries for the Mercy House program. When possible, provide 3 referrals for other programs in the event Mercy House is unable to assist with residency.
- Oversee and assist with maintaining a referral list of programs for incoming inquiries that do not fit our program.
- Assure that a record of calls and online inquiries is kept and documented in the Resident Care Report (Excel)
- Prepare a monthly resident care report for board meetings outlining each resident’s progress and needs, maintaining resident confidentiality, and including prayer requests.
Resident Intake Process
- Assist with the application process for new inquiries when needed.
- Conduct applicant interviews (by phone, via Zoom) with potential new residents.
- Assess the ministry’s ability to meet applicant needs with the Executive Director.
- Facilitate background checks.
- Oversee and facilitate the resident intake process for all new residents.
- Assure that all forms and releases are signed and dated before the resident enters Mercy House (via DocuSign)
Service Plan Implementation
- Define each resident’s needs and barriers, including, but not limited to, spiritual, medical, life skills, education, mental health, job training, and family healing. Create an individual service plan with each resident within 10 days of their admission to Mercy House.
- Meet with residents regularly (weekly recommended) to discuss and implement the service plan.
- Maintain case notes of resident progress in the service plan in a timely fashion.
- Determine goals or subsequent disciplinary actions for non-compliance with service plan action points—document plans and warnings for lack of compliance.
- Assure that resident charts are correctly maintained and completed upon graduation or discharge of a resident.
Program Exit
- When possible, conduct exit interviews with residents in cooperation with the Executive Director and House Manager and document accordingly.
- Complete the resident chart upon graduation or discharge from the program.
- Assist with decisions that are not able to be assessed with standard processes.
- Assist with the discharge of residents from the program or other disciplinary actions needing attention.
Support
- Allow for complete anonymity regarding case management discussions & instill trust in the resident’s decision-making ability.
- Pray for her family (express genuine interest & care for specific family issues)
- Send encouraging text messages (via Google Voice number)
- Make regularly scheduled phone calls/discussions on areas of improvement needed
- Be available to residents to build a trusting relationship
- Collaborate with residents, staff, and volunteers
- Provide resident transportation
Program Development
- Update the resident resource guide for the state of Texas, identifying resources for housing, transportation, mental health, etc.
- Updateonboarding training to include conflict resolution, incident reporting, see-something-say-something, confidentiality, etc.
- Work with Mercy House Intern for an overlap period to train on our processes and develop additional resident resources.

Job Type: Part-time
Pay: $18.00 - $20.00 per hour
Expected hours: No more than 25 per week
