Schedule: Monday-Friday, 8:00 a.m. - 4:30 p.m. OR 8:30 a.m. - 5:00 p.m.
Job Summary:
The Housing Solutions Case Manager provides ongoing needs assessments, treatment plans, support, referrals, and advocacy for residents in the Mercy House Permanent Supportive Housing Program. This role fosters self-reliance through positive relationships with residents, addressing barriers to housing stability, and maintaining clear, accurate records. The Case Manager ensures the team uses trauma-informed, client-focused strategies.
Essential Duties and Responsibilities:
Client/Service Delivery
- Manage a caseload of 10-25 residents, meeting at least twice a month to monitor stabilization plans.
- Use harm reduction methods to support housing stability.
- Apply crisis prevention and intervention strategies as needed.
- Practice Trauma-Informed Care in client interactions.
- Assist residents in identifying strengths and needs.
- Inspect resident apartments to ensure proper maintenance.
- Refer clients to community resources as needed.
- Organize activities to promote community involvement and peer support.
- Connect residents with services such as employment, education, and mentoring.
- Attend training to enhance knowledge of client needs (e.g., co-occurring disorders, harm reduction, cultural competence).
- Facilitate relationships between residents and landlords.
- Participate in the emergency on-call schedule.
Team/Staff Relations
- Collaborate with staff and volunteers to implement Mercy House’s mission and core values.
- Attend internal and external agency meetings.
- Contribute to the planning and development of resident programs.
Administration
- Maintain accurate, up-to-date client documentation, including stabilization plans and case notes.
- Complete paperwork promptly to ensure timely payments to the agency.
- Assist with data collection and reporting systems.
- Willingness to learn about Adult Rehabilitative Mental Health Services and Substance Abuse Treatments.
- Perform other duties as assigned.
Requirements:
Education and Experience
- Bachelor’s Degree in Social Work or a related field, with experience in chemical health, mental health, or co-occurring disorders preferred.
- Candidates with at least 2 years of experience working with long-term homeless, low-income, and diverse populations, and knowledge of mental health issues, are encouraged to apply.
General
- Ability to work with a diverse population, prioritize tasks, and perform crisis intervention.
- Strong communication skills, both written and verbal.
- Maintain a positive, professional environment while establishing effective relationships.
- Dependable and responsible, with the ability to communicate effectively and respectfully.
- A valid California driver’s license, auto insurance, and reliable vehicle required.
Physical Requirements
- Ability to lift up to 50 pounds.
- Prolonged periods of sitting at a desk and working on a computer.
