Schedule: Monday-Friday, 9am-1pm OR 1pm-5pm
- This is a PART-TIME position (expected 20 hours/week).
- Weekends, evenings, and/or availability to respond to crisis situations may be required depending on programmatic needs.
Job Summary:
The Housing Solutions Case Manager is responsible for assessing needs, creating and implementing treatment plans, providing support, referrals, and advocacy for individuals in the Mercy House Permanent Supportive Housing Program. The role requires fostering strong relationships with residents to enhance their self-reliance and stability, recognizing positive behavioral changes, and addressing challenges to self-sufficiency. Clear documentation and trauma-informed, client-focused strategies are essential.
Essential Duties and Responsibilities:
Client/Service Delivery
- Manage a caseload of 15 individuals, meeting at least twice a month to develop and monitor stabilization plans.
- Use harm reduction methods to maintain housing stability.
- Apply crisis prevention and trauma-informed care techniques.
- Support residents in identifying strengths and needs respectfully.
- Inspect resident apartments for proper maintenance.
- Refer clients to community resources as needed.
- Organize social and recreational activities to promote community involvement.
- Link residents with services such as employment, education, and mentoring.
- Attend training to improve service delivery, including co-occurring disorders and cultural competence.
- Facilitate relationships between residents and landlords.
- Participate in the emergency on-call schedule.
Team/Staff Relations
- Collaborate with Mercy House team members to implement the agency’s mission and values.
- Attend internal and external meetings.
- Contribute to the development of resident programs.
Administration
- Maintain accurate documentation (stabilization plans, case notes, etc.).
- Complete paperwork promptly to ensure timely agency payments.
- Assist with data collection and reporting.
- Gain knowledge of Adult Rehabilitative Mental Health Services and Substance Abuse Treatments.
- Perform other duties as assigned.
Requirements:
Education and Experience
- Bachelor’s Degree in Social Work (or related field) with experience in chemical health, mental health, or co-occurring disorders preferred.
- Candidates with at least 2 years of direct experience working with long-term homeless, low-income, and diverse populations, and knowledge of mental health issues, are also encouraged to apply.

- Ability to work effectively with diverse populations, plan and prioritize tasks, perform crisis intervention, and communicate clearly in both written and verbal forms.
- Maintain a positive, professional, and safe environment, and establish effective working relationships.
- Dependability, responsibility, and effective communication are essential.
- A valid California driver’s license, auto insurance, and reliable vehicle required.
Physical Requirements
- Ability to lift up to 50 pounds.
- Prolonged sitting at a desk and working on a computer.
