Job Summary:

Community Housing Advocates (CHA) is looking to a highly skilled and experienced case manager to serve as a Community Social Worker (CSW) floater for CHA’s two affiliate agencies, Mercy Housing and Shelter and My Sisters’ Place. This person will help ensure that we are able to continue to meet agency goals and provide high quality services to our participants when turnover occurs within the agency.

The Floating CSW’s responsibilities include temporarily covering positions across departments and programs as necessary to ensure continuity of service provision, focusing on the essential duties of each position. While specifics will vary by position, conducting assessments, person-centered goal planning, and documentation of services provided will broadly be required for all positions. Additionally the CSW provides linkages to external medical, mental health, and substance abuse resources, entitlement/state benefits assistance, and employment opportunities.

Scheduled Work Times/Locations

Monday – Friday, 8:30AM – 4:30PM; flexibility for occasional nights and weekends when necessary

· Are flexible and able to take direction from different people from day to day and work out of different locations

· Have a strong understanding of how to work collaboratively with program participants to identify their goals and incorporate them into service plans which will allow clients to thrive in their communities.

· Have excellent case management documentation skills and a solid grasp on client data entry systems.

· You thrive in environments that require learning new things, getting to know new people and being challenged by new opportunities

· Have excellent time management skills with a proven ability to meet deadlines; ability to prioritize tasks

· Have strong interpersonal skills with the ability to communicate effectively both verbally and written with diverse clientele.


Duties/Responsibilities:

Depending on the specific role being covered examples of duties include:

· Provide intake/orientation to new clients.

· Assess clients’ their strengths, needs, abilities and preferences and create person centered service plans which will inform case management services provided to allow clients to reach their goals.

· Provide case management services including assessment and person-centered service planning, budgeting and financial literacy, employment search, access to entitlements, and benefits.

· Linkage to other community supports, including behavioral or medical health services, childcare, volunteer opportunities, mainstream resources, basic needs, transportation, etc.

· Advocacy and monitoring of service plan to reduce barriers and increase client integration into their communities.

· Assist program participants with housing search and lease up processes, and applying for flexible financial resources as necessary

· Coach clients on their rights and responsibilities as program participants, tenants and other roles they may fill in the community

· Complete all required documentation with accurate and timely entry into HMIS including case notes.

· Create and maintain consistent communication channels, both verbal and written between tenants, landlords, referral sources, debtors, creditors and other collaborating agencies or individuals.

· Transport clients as deemed necessary-Limiting transport service plan related goals.

· Attend provider meetings as assigned.

· Assume other responsibilities as assigned.


Education and Experience:

· Master’s degree in Social Work, or Human Services field from an accredited university, however equivalent education and experience will be considered.

· 3-5 years relevant work experience and demonstrated ability to provide effective services to clients required.

· Knowledge of homeless and recovery programs available in Greater Hartford is preferred.

· Bilingual (English/Spanish) candidates strongly encouraged.


Licenses and Certifications:

· Must have valid Connecticut driver’s license and clean driving record with proof of automobile insurance. Staff will be expected to transport clients in their personal car.


Compensation & Benefits

· $26-$28 per hour, based on experience plus generous incentive compensation plan


· Community Housing Advocates provides a generous employee benefits package, including medical, dental, retirement, short- and long-term disability, and tuition reimbursement. Full-time employees receive 10 days of PTO (paid time off) per year, plus 11 paid holidays, 1 Floating Holiday, and up to 40 hours of accrued paid sick leave.
