What are the responsibilities and job description for the Community Liaison/Social Worker position at Mercy Ridge Inc?
Mercy Ridge, an upscale retirement community located in Timonium, Maryland is seeking a full time Community Liaison/Social Worker to become a part of our team. Under the direct supervision of the Director of Life Enrichment, the incumbent is responsible for assisting Independent Living and Assisted Living residents to achieve and maintain maximum functioning in and outside the community through needs assessment, crisis intervention, education and counseling. This position is also responsible for program development and implementation to meet the needs of the eight (8) dimensions of wellness.
ESSENTIAL FUNCTIONS:
- Welcome new residents to community by providing initial support after move in and while living in the community
- Maintain excellent communication with residents, families, physicians, nursing staff, and referral sources
- Provide active listening therapy with residents through behavioral and interventional counseling as needed
- Encourage residents to participate in individual and group programs and to utilize these programs to full advantage
- Assist residents and families in navigating through the continuum of services and levels of care
- Support residents and family members in dealing with challenges of aging
- Coordinates the monthly Health Maintenance meetings to identify issues or concerns regarding residents in Independent Living.
- Facilitate the Resident Council meetings for Assisted Living
- Manages the Motorized Ambulatory Aid process to ensure each resident meets the requirements to operate a MAA in the facility
- Acts as resident advocate and participates in family/resident meetings
- Participates on committees as assigned
QUALIFICATIONS:
- A Master’s degree in Human Services or Social Work or a Licensed Social Worker
- High level of interpersonal skills to handle sensitive and confidential information and to interact with all levels of the organization
- Ability to communicate effectively, able to read, write and follow written and verbal directions
- Knowledgeable in federal, state and local regulations
