Primary Care Patient Care RN
This position requires travel to primary care clinics in Fort Smith and surrounding areas.
Overview:
The Primary Care Patient Care RN works under the direction of the primary care operations team to provide safe, timely, effective, efficient, equitable, patient-centered care. The RN partners with patients, providers, and other clinical team members to drive quality outcomes for all patients.
Duties and Responsibilities:
- Collaborate with Mercy Primary Care Providers to deliver high-quality patient care.
- Partner with patients to develop and implement individualized care plans.
- Communicate effectively with the clinical care team to ensure seamless transitions and continuity of care.
- Maintain accurate and up-to-date patient records.
Qualifications:
- Education: Associate of Applied Science or Bachelor's Degree in Nursing.
- Licensure: Active RN licensure.
