What are the responsibilities and job description for the Patient Navigator position at Mercy?
Description: We are seeking a skilled Navigator to join our vAcute team at Mercy Virtual in Chesterfield, Missouri. This is a full-time position working 36 hours per week with 3/12 hour shifts from 6:30 AM - 7 PM, rotating weekends and holidays.
The vAcute team is a collaborative group of navigators, RNs, Advanced Practice clinicians, and a board-certified Emergency Medicine physician who work together to take care of patients outside of the hospital setting. Through various access points, patients can reach the team 24/7 to address medical issues in real-time. Our team works closely with primary care physicians and specialists to optimize patient experience, increase communication, and ensure the most appropriate level of care.
Job Responsibilities:
- Facilitate patient/family experience by providing coordinated, personal, professional, and compassionate service.
- Answer telephone calls from patients/family in the Mercy Virtual vAcute Program.
- Perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Requirements:
- Experience: Previous experience in a Medical Office, Emergency Department, or Hospital.
- Education: High school diploma.
- Certifications: BLS required or within 6 months of hire.
- Skills: Critical thinking and problem-solving skills; professional verbal and written communication skills; ability to work in triage software and electronic medical record.
