About the Role
- We are seeking a skilled Registered Nurse or healthcare professional to join our interdisciplinary team.
This role involves working with our team to provide comprehensive physical medicine and rehabilitation programs to patients throughout their recovery journey. You will work closely with our patients, families, and caregivers to develop and implement individualized treatment plans tailored to their unique needs.
Your Responsibilities
- Completes departmental orientation, initial and annual competencies.
- Consults with other departmental staff to collaborate in patient care delivery and identify barriers to care and discharge.
- Completes documentation per workflow timeline and content requirements, including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
- Schedules family conferences and communicates with caregivers following each team conference to keep patients and designated caregivers informed of progress.
- Coordinates weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans.
- Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions.
Requirements
- Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
- Certification in Case Management or Rehabilitation Nursing preferred.
- Minimum of 2 years social work or case management experience in an inpatient setting highly preferred.
- Effective oral and written communication skills in English, additional languages preferred.
