Proof of vaccine Sept 2024 booster, declination or exemption

Attached scanned proof of 2 vaccinations with dates or primary source IgG titer

Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later

Proof of vaccine, declination or exemption

Attached scanned proof of 1 vaccination with date or primary source IgG titer

Upload a current, non-expired TDAP vaccine

Provide documentation from an exam within the last year that candidate is fit to work

Resume, Skills Checklist within a year, 2 Professional References

Federal, Nationwide, County Criminal from last 7 years

Attach completed 3rd Party Screening Company report of candidates highest level of academic education

Attach scanned EPLS report from website or result provided by third party screening company

Attach scanned OIG result print-out from the website or result provided by third party screening company

Must be dated within 30 days of start

Must be completed and uploaded as soon as possible
