Licensed Social Worker!

Benefits:Licensed Social Worker Job Requirements:
- Experience as a social worker within a nursing home or rehabilitation center strongly preferred
- Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities
- CT Social Work License (Required)
- Admission: 1 year (Preferred)
- Long term care: 2 years (Preferred)
- Plan, organize, development and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
- Performs all documentation requirements for social service documentation in a timely fashion.
- Completes all admission assessments and evaluations
- Works with the interdisciplinary team to create a plan of care for residents.
- Works with and provides support to residents, families and staff.
- Attends weekly Care Plan and Medicare meetings.
- Receives and attempts to resolve resident grievances.
- Coordinates resident discharge planning with all relevant departments and assists with scheduling outside services.
- Participates in monthly team meetings such as QA and Safety.
