This position is grant-sponsored and benefit-eligible, with a fixed term concluding on August 30, 2025, upon the expiration of the grant.

- Track and tally required services provided.
- Meet quality and quantity of services benchmarks.
- Record every activity engaged in on behalf of a client daily in electronic health records.
- Research, develop, and maintain a comprehensive list of community resources that includes addresses, phone numbers, and names of contacts for each resource.
- Perform all other duties as required to meet program and departmental goals.
Other Duties/Responsibilities:
- Table outreach events. Provide monitoring of youth/children when requested by parents during parent's involvement, engagement, discussion, and applications/processes for resources/entitlements.
- Assist with and provide service as outlined in the development of the community Outreach Plan of Action to assist in meeting deliverables of grant.
- Link citizens with services for children or special needs citizens and resources identified in the Community Plan.
- Maintain contact with all staff/agencies providing services to monitor clients' progress and assess appropriateness of services.
- Serve as a resource to citizens with services for children or citizens with special needs and other community agencies.
Who are we looking for?
Education: High school diploma or GED equivalent required. Associate of Arts Degree, BA/BS in Human Services field/focus of study preferred.
Experience: 1 year experience in Care Management with primary care, Clinical Support System for Serious Mental Illness, (SMI) or similar populations (may include experience in case management, community organizing, or working with local organizations)
Drivers Requirement:
Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
In order to qualify as a Center Driver, the candidate must be at least 21 years of age and have an active Florida Driver's license. Motor Vehicle Record will be reviewed for eligibility. No more than 3 moving violations and/or accidents within the past 3 years and no major violations during the past 5 years.
