Job Summary
This remote role involves supporting care management activities and ensuring continuity of care for members. You will serve as a liaison between care managers, providers, and members to facilitate proper coordination of care.
Key Responsibilities
- Engage members through phone or home visits to discuss care plans, resources, and ongoing education.
- Coordinate care activities based on member needs and work with healthcare providers to accommodate changes or progress.
- Serve as a support point for member inquiries, requests, or concerns related to care plans.
- Develop in-depth knowledge of care management services, including responding to complex issues.
- Communicate with care managers, practitioners, and others to ensure continuity of care.
- Perform service assessments and document member records according to state and regulatory requirements.
- Work with the care management team to triage, adjust, and escalate complex requests.
- Follow standards of practice and policies compliant with contractual requirements and regulatory guidelines.
Requirements
- Bachelor's degree in nursing, social sciences, social work, or related field.
- One year of supervised clinical experience in a human-services field.
